What to include
Area, provider, dates, reference and service tests that do not reveal private account details.
Add a brief, factual account of the impact and the outcome you want. Use dates and public references where possible.
What to leave out
Leave out other people’s names, unnecessary health details, PPS numbers, bank details and private documents. If an office needs sensitive identifiers, ask for an appropriate private channel.

What happens next?
Your email app opens a draft for you to review and send. Keep a copy. Ask the office what the next step will be and follow up on the specific question if you need to. A TD reply is not a decision by the service responsible.
Use the official service channel for the complaint or application itself. Check its current instructions for any time limit; do not wait for a TD reply before a deadline.
