Shannon Valley Plant Hire was fined €600,000 at Dublin Circuit Criminal Court in March 2024 after pleading guilty to a workplace safety offence linked to Christopher Miley’s death.
Miley, a Euro Oil driver, was struck by a loader shovel at Behan Quarry in Rathcoole, Co Dublin, while refuelling in darkness.
A later inquest heard that the quarry had no lighting for work outside daylight hours and that the reversing machine’s sensor had a blind spot.
The inquest jury recorded a verdict of death due to an industrial accident and reiterated concerns about lighting and refuelling procedures.
