A HSA inspector has criticised a “laissez-faire approach” to health and safety by the Irish branch of engineering consultancy giant Ove Arup after the death of a man who fell 24 metres down a ventilation shaft at their Dublin office. John McCann (62) fell to his death through an inadequately signed
