The Charleton Tribunal received three of four requested statements on 5 October 2017, according to a report published that day.
Statements from Chief Superintendent Terry McGinn, Chief Superintendent Tony McLoughlin and Superintendent Eugene McGovern were received by the inquiry. Retired assistant commissioner Kieran Kenny could not provide a statement at that time because he was abroad.
The tribunal had requested the statements after hearing about handwritten notes taken at a Garda conference in Letterkenny Station on 8 October 2013. The material concerned what the officers knew about that meeting and contacts involving Garda Keith Harrison and Marisa Simms.
The report said Garda headquarters had not initially forwarded Chief Superintendent McGinn’s notes to the tribunal because they did not appear to refer to Tusla or the HSE. Garda Commissioner’s counsel regretted the documentation error and said processes were being reviewed.
