In March 2016, 48‑year‑old Boguslaw Szbisty, a contract worker for Key Waste Management, was crushed by a Manitou Telehandler while sorting waste at the company's Dublin site. The incident occurred after the telehandler driver mistakenly believed the workers had finished and began reversing. CCTV footage showed the vehicle operating close to the workers and colliding with Szbisty, who was later pronounced dead. An investigation by the Health and Safety Authority found Key Waste had failed to segregate pedestrian workers from the telehandler, and the vehicle lacked side and rear‑view mirrors and had an expired safety certificate.
